This interactive online program gives you the information, motivation, and support you need to take charge of your health. You are eligible to earn a $50 cash reward for each 12 weeks you participate in one of the programs. whether you want to lose weight, manage stress, quit smoking, or get fit, Healthy Lifestyle Rewards can help you get there. To enroll in Healthy Lifestyle Rewards, you will need to be registered on Blue Shield of California's website. First, go to www.blueshieldca.com/hlr. If you are not already registered on blueshieldca.com, click "Register Now". If you are already registered on blueshieldca.com, enter your username and password. Read the program participation terms, then click "I agree." You'll be directed to your Personal Home Page for Healthy Lifestyle Rewards. At your Personal Home Page, click on the Wellness Assessment tab at the top of the page, and answer the questions. The Wellness Assessment gives feedback about how your lifestyle affects your health and highlights where improvement would increase your well-being. When you finish the assessment, click on Choose Your Program to select the program that's right for you. On your first visit, you'll be asked to pick a program that interests you.
